At the end of every semester we request that students fill out an anonymous evaluation form for each graduate course that they have taken within the astronomy department, to help guide the faculty in improving course materials and presentation.
We provide the blank forms here, in
postscript format,
so that you may print them out at any time that you wish to fill them in.
